Understanding a Natural Hairline

A natural hairline is what most clients want, but not every clinic/hospital can do it. Creating new hairline to mimic a natural hairline needs good practice and technique. Our surgeon, Dr Pukpinya Jangjetriew, has described as following.

1. Appropriate Graft Density

On the recipient area, the surgeon will create a density of 40-50 grafts/sq.cm, which is a worldwide standard for a hair transplant. However, the surgeon will also have to consider other factors before selecting the ideal density for the client. Some people walked in with very severe alopecia.

In this case, the donor will not be enough to place to the whole area with the standard density. The surgeon will have to adjust the density of the hair and manage the donor area for the best and balanced outcome.

2. Creating a Natural Hairline

Apart from the density, the surgeon will also have to create a natural hairline for the client. The surgeon does this by placing the one-hair graft in the few front rows and placing the two-hair or three-hair graft further to the back. This will create a gradient of the thickness of the transplanted hair. If the hairline starts thick, it will look too rigid and unnatural. You can have a look at your hairline before a transplant. You can see that the hairline starts off being thin, then gradually gets thicker.

3. Irregularity of the Hairline

If you look at your hairline closely, you will see that your hairline is not exactly straight. When the surgeon draws the hairline, she will also try to make it irregular to mimic the natural hairline. If we make it too straight, it will look like you are wearing a wig, which looks fake.

4. Appropriate Hairline Height

The height of the hairline is also important to frame your face. Too high, your forehead will look big, and your face will look longer. Too low, your hairline will move when you lift your eyebrows, and your face will look shorter. The height from your chin to the tip of your nose and the height of your forehead should be a 1:1 proportion.

5. Direction of Hair

The direction of the hair is the hardest thing to correct once the transplant is complete because the surgeon will have to extract the hair follicle out and place it back in again one by one. It could make a permanent scar on the recipient area and create thick fibrosis under the skin and lead to a decrease in the survival rate of the transplanted hair.

6. Caring for the Donor Area

The donor area (the back of your head) is another area that the doctor has to be careful and aware of the natural look. This is because the hair will not grow back on the area that the doctor extracts the follicle out. If the doctor harvests the follicle too close to each other, it will create a bald spot or uneven thinning area.
